What is an epidermoid cyst?
An epidermoid cyst is a slow-growing sac beneath the skin filled with keratin, the same protein found in skin, hair and nails. These cysts are frequently called "sebaceous cysts," although that name is usually inaccurate because most do not originate from sebaceous glands.
Epidermoid cysts commonly develop on the face, neck, back, chest and trunk. Pilar cysts are similar but typically occur on the scalp.
A cyst may remain small and painless for years. Treatment becomes more reasonable when it:
- Continues to enlarge
- Becomes painful, red or inflamed
- Repeatedly drains or develops an odor
- Rubs against clothing or interferes with activity
- Causes cosmetic concern
- Returns after previous drainage
- Has an uncertain diagnosis
Not every lump beneath the skin is a cyst. Lipomas, swollen lymph nodes, abscesses and other soft-tissue masses can have a similar appearance. A medical evaluation is important before deciding how a lump should be treated.
Cyst removal versus cyst drainage
Cyst removal and cyst drainage are not the same procedure.
Complete cyst excision removes both the contents of the cyst and its surrounding wall, sometimes called the capsule or sac. Removing the entire cyst wall reduces the likelihood that the cyst will return.
Drainage opens the cyst and releases its contents but may leave the cyst wall behind. Drainage can provide rapid relief when a cyst is acutely swollen, ruptured or infected, but the remaining wall can allow it to refill later.
An inflamed or infected cyst is sometimes too fragile to remove completely during the initial visit. In that situation, the safest approach may be to drain the cyst, treat the inflammation or infection, and consider definitive excision after the area has healed.
Is an abscess the same as a cyst?
No. A skin abscess is a pocket of pus caused by infection. It is often painful, warm, red and tender and may enlarge quickly.
The primary treatment for many abscesses is incision and drainage. After numbing the area, the physician makes a small opening, drains the infected material and cleans the cavity. Antibiotics may also be prescribed when clinically appropriate, such as when there is surrounding cellulitis, systemic illness or another risk factor.

Can urgent care remove a cyst?
Many traditional urgent care centers drain abscesses but refer patients elsewhere for complete cyst removal. KaufCare was designed to provide a broader range of advanced procedures on site.

What happens during same-day cyst removal?
The exact procedure depends on the cyst's size, location and degree of inflammation.
First, the physician examines the area and confirms that office-based removal appears appropriate. When the diagnosis or depth is uncertain, bedside soft-tissue ultrasound may help distinguish a cyst from an abscess, solid mass or nearby blood vessel.
The skin is then cleaned and numbed with local anesthetic. You may briefly feel the anesthetic injection, followed by pressure or movement during the procedure, but the area should not feel sharp pain.
For complete excision, the physician makes an incision over the cyst, carefully separates it from the surrounding tissue and removes the cyst wall whenever possible. The incision is then closed with sutures and covered with a dressing.
Most patients return home immediately after the procedure. Depending on the location, you may need to limit strenuous activity while the incision heals and return for a wound check or suture removal.

Most epidermoid and pilar cysts are benign. However, visual examination alone cannot provide the same confirmation as microscopic evaluation.
After excision, KaufCare sends the specimen to a pathology laboratory. A pathologist examines the tissue to confirm the diagnosis and identify any unexpected findings. This provides an additional layer of certainty, particularly when a mass has grown, changed or repeatedly returned.

Should you pop or drain a cyst at home?
Do not cut, puncture or attempt to remove a cyst yourself. Home treatment can push inflammation deeper, introduce bacteria, cause bleeding and increase scarring. Squeezing out the contents also leaves the cyst wall behind, making recurrence likely.
Warm compresses may temporarily help a small inflamed area, but an enlarging, painful or draining lump should be evaluated by a medical professional.
When should a cyst or abscess be evaluated promptly?
Seek prompt medical care if you develop:
- Rapidly increasing pain, redness or swelling
- Fever or chills
- Red streaking extending from the area
- Foul-smelling or heavy drainage
- Significant swelling near the eye or face
- A rapidly growing, hard, fixed or irregular mass
- Recurrence after previous removal
- Symptoms of infection while immunocompromised
Severe illness, confusion, fainting, rapidly spreading infection or concern for a life-threatening emergency warrants emergency-department evaluation.
Frequently asked questions about cyst removal
Can a cyst be removed during the first visit?
Often, yes. Many uncomplicated epidermoid and pilar cysts can be removed during a same-day visit. An acutely infected or severely inflamed cyst may need drainage first, with definitive excision considered after the inflammation resolves.
Does cyst removal hurt?
The area is numbed with local anesthetic. Patients typically feel a brief sting during the injection and pressure during removal, but should not feel sharp pain.
Will the cyst come back?
Recurrence is less likely when the complete cyst wall is removed. A cyst that is only drained can refill because its wall remains beneath the skin.
Do antibiotics remove a cyst?
No. Antibiotics do not remove the cyst wall. They may be used when there is a bacterial infection or surrounding cellulitis, but drainage or excision may still be necessary.
Is the tissue sent to pathology?
