Types of cysts, their common names and characteristics

• Epidermoid cyst — also called sebaceous cyst, epidermal inclusion cyst, keratin cyst, “wen”

  Firm, movable lump on the face, neck, chest, back or earlobe, often with a small central pore. Treated by complete excision of the cyst and its sac.

• Pilar cyst — also called trichilemmal cyst, scalp cyst

  Smooth, firm scalp bump, frequently multiple and often familial. Excised with the capsule intact.

• Ganglion cyst — also called wrist cyst, “Bible bump”, hand or knuckle lump

  Rubbery swelling on the wrist, hand or top of the foot filled with clear jelly-like fluid; it may change size. Treated by ultrasound-assisted aspiration.

• Digital mucous cyst — also called mucous cyst, myxoid cyst

  Small dome-shaped bump at the last finger joint or nail fold, linked to osteoarthritis. Aspiration and hand-surgery referral when the joint is involved.

Cyst removal versus cyst drainage

Drainage empties the contents through a small opening and relieves pressure quickly, but the sac remains, so the cyst commonly refills. Excision removes the sac and cyst wall, which is what prevents recurrence. An infected cyst is drained first and excised after the inflammation resolves. Lumps that are not cysts — lipomas, milia, hidradenitis suppurativa nodules and enlarged lymph nodes — are evaluated with bedside soft-tissue ultrasound when the diagnosis or depth is uncertain.
